Case Manager

As our Case Manager, you will engage the client in creating and implementing an individual service plan, identifying their self-identified goals and objectives, service needs, strengths, barriers, support systems, and emotional triggers. Using this plan, you will link the client to community services and work with them directly to provide support and training on living skills such as grocery shopping, budgeting, home upkeep, meal planning, social skills, coping strategies, and self-monitoring their psychiatric symptoms. Through regular home visits, you will monitor the clients progress and behaviors, and support the clients goal attainment. This position includes client transportation as needed. For optimal client success, you will primarily meet with clients in the local community.

What we are looking for:

  • Bachelors Degree in Social Work or Psychology or a related field
  • Experience working in the mental and behavioral health field
  • Professional and courteous communication (verbal and in writing) and knowledge of and sensitivity to culturally diverse and special needs populations.
  • Valid Georgia Drivers License and clean Motor Vehicle Record (MVR);
  • Clean Criminal Back Ground Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation-child or adult)
