The Clinical Care Coordinator- Certified is the key liaison between the patient and their Patient-Centered Medical Home team. Team includes providers, clinic staff, and other health care professionals. Clinical Care Coordinator works to ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.

Required qualifications:

  • Completion of an accredited Medical Assistant program.
  • Upon hire: National Medical Assistant Certification
  • Upon hire: National Provider BLS - American Heart Association
  • Upon hire: Obtain appropriate certification(s) within designated time frame for a particular field of practice (for Specialty areas)
  • Within 1 year of hire: Audiometric, spirometry and EBT (evidential breath alcohol) testing to possess or gain the appropriate certifications (for Occ Health)
  • 3 years Recent health care experience working as a Medical Assistant.
  • 6 months Specialty areas: Recent experience working as a Medical Assistant.
  • Cardiovascular Care: Experience in a cardiovascular practice.
  • Oncology: Experience in Oncology practice.
  • Dermatology: Experience in Dermatology practice.
  • Pediatrics: Experience in peds practice.
  • Women's Health: Experience in women's health practice.
  • Neurological Services: Experience in Neuro practice.
  • Thorasic Surgery: Experience in Thorasic Surgery practice.
  • Arthritis: Experience in Arthritis practice.
  • Infectious Disease: Experience in Infectious Disease practice.
  • Colorectal: Experience in Colorectal practice.
  • Physiatry: Experience in Physiatry practice.
  • Ortho: Experience in Ortho practice.
  • General Surgery: Experience in General Surgery practice.
  • Plastics: Experience in Plastics practice.
  • ENT: Experience in ENT practice.
  • Pharmacy: Experience in Pharmacy practice.
  • TeleHealth: Experience in TeleHealth practice.
  • Pulmonary: Experience in Pulmonary practice.
  • Informatics: Experience with computer-based technology and healthcare applications to manage and coordinate patient care.

Preferred qualifications:

  • College.
  • Experience with electronic medical records and/or computerized billing systems.
