Job Description

Overview


The Physician Liaison, under the Director of the Executive Vice President and Chief of Patient Care Services, utilizes the nursing process to develop, implement, and evaluate outcomes for defined patient populations. This individual utilizes a variety of skills to educate, consult, and assist in patient management of an identified patient population to achieve the highest quality and best possible outcome for the patient in a cost-effective manner. The Physician Liaison collaborates with a patient care team that includes, but is not limited to physicians, nurses and assistive staff, rehabilitative therapy, respiratory therapy, pharmacy, dietetics, any other disciplines directly and indirectly involved inpatient care, identifying opportunities for the improvement of patient care/outcomes and cost reduction based on current research findings supported in the literature. He/she supports the mission, vision, and values of Floyd Medical Center and contributes to the accomplishment of the purpose and objectives of Floyd Medical Center.


QUALIFICATIONS


EDUCATION REQUIRED


Graduate of an accredited school of Registered Professional Nursing


EDUCATION PREFERRED


Bachelor of Science in Nursing


LICENSURES OR CERTIFICATIONS REQUIRED


Current license to practice as a registered nurse in the State of Georgia


LICENSURES OR CERTIFICATIONSPREFERRED


Participation in specialty organization

Certification in specialty


EXPERIENCE REQUIRED


At least ten years working in the care of assigned patient population

ROLE SPECIFIC ESSENTIAL DUTIES AND RESPONSIBILITIES


Provides clinical expertise in defined area of specialty:


Coordinates and provides direct patient care as indicated by the clinical situation, collaborating with physician staff to promote wellness and prevent illness.

Responds to healthcare team members' requests for intervention in complex patient needs.

Revises and adjusts care plans to meet individual patient needs, related to specialty.

Involves patient and family unit in goal setting and communicates with patient and family about plan of care and discharge goals.

Participates in discharge planning.

Collaborates frequently with physicians regarding patient progress and needs.

Rounds with providers to discuss patient progression.

Collaborates frequently with Directors in the evaluation of supplies and equipment that support service line.

Collaborates with utilization managers to identify customers within service lines to assess needs, plan interventions, implement activities, and evaluate outcomes that exceed customer expectations.

Leads SNAP Huddles on each nursing unit to anticipate barriers to discharge and help remove them.

Escalates barriers to discharge as needed.

Holds the care team accountable for meeting the patient's anticipated date of discharge.

Performs other related duties as assigned.


Utilizes and participates in research opportunities in defined area of specialty:


Facilitates the evolution of critical pathways based on scientific findings supported by research literature.

Tracks outcome performance data and significant outcomes to assess current clinical practice in relating to research based on literature.

Plays leadership role in quality improvement activities for identified patient populations using a multidisciplinary model.

Contributes to clinical research studies applicable to designated patient populations.


Participates in the development and utilization of educational resources available to patients, families, and staff within the defined area of specialty:


Identifies education/marketing needs and coordinates with patient care team to develop programs and materials for patients, family units, and the community.

Works directly with staff to develop necessary skills and appropriate documentation tools.

Improves knowledge base through attendance and participation in continuing education programs, completes education that is age related to specialty, completes continuing education in leadership.

Collaborates with Directors in identifying staff learning needs and provides instructional support as indicated.

Collaborates with appropriate customer groups in educational strategic planning for organization, community, and business partners and provides instructional support and clinical expertise as appropriate.


Demonstrates administrative characteristics intrinsic to the role within defined area of specialty:


Promotes the mission, purpose, objectives and strategic planning of Floyd Medical Center.

Collaborates with healthcare team through PI process to enhance efficiency cost effectiveness and clinical outcomes.

Assists in evaluation and revision of policy and procedure guidelines and standards of care and practice in cooperation with health care team.

Communicates appropriately with patients and families from neonatal, child, adolescent, adult and geriatric populations.

Provides for age appropriate environment, education and procedures in area of specialty.

ENVIRONMENTAL AND JOB HAZARDS
Frequent exposure to communicable disease, bodily fluids and tissue, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinical environment.

TYPICAL PHYSICAL DEMANDS
Full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Standing and walking for extensive periods of time. Occasionally lifts and carries weight up to 50 pounds. Ability to distinguish letters, numbers and symbols with vision and hearing within normal range to respond to Code alerts and equipment alarms. Work-related stress due to volume and time constraints with irregular working hours.
